Eines der medizinischen Felder, in dem die ethische Diskussion um die „wunscherfüllende Medizin“ am intensivsten geführt wird, ist die Reproduktionsmedizin, die die Erfüllung des „Kinderwunsches“ verspricht. Strittig ist besonders, ob Sterilität als Krankheit definiert wird, die eine medizinische Intervention rechtfertigt, ob sich aus der Sterilität oder Infertilität lediglich ein Abwehr- oder auch ein positives Anspruchsrecht auf medizinische Ressourcen ergibt, ob legitime Fortpflanzungsmedizin Grenzen hat. Nach einer Übersicht über Eckpunkte der nationalen und internationalen Debatte beschreiben wir im zweiten Teil Ansichten zum (...) Status ungewollter Kinderlosigkeit, zum Recht auf Fortpflanzungsmedizin und zur möglichen Legitimität ihrer Begrenzung anhand der Ergebnisse unserer vom Bundesministerium für Bildung und Forschung (BMBF) geförderten Studien. Während der letzten fünf Jahre haben wir u. a. Paare mit und ohne Sterilitätsproblematik sowie verschiedene Expertengruppen qualitativ und standardisiert befragt. Die stark divergierenden Auffassungen konfrontieren wir im dritten Abschnitt mit internationalen Standards, zum anderen mit dem „Capability-Ansatz“ und dem freiheitsfunktionalen Liberalismus von Sen und Pauer-Studer. Dies ergibt, dass die Bereitstellung verschiedener Möglichkeiten der In-vitro-Fertilisations-(IVF-)Behandlung nicht lediglich einen Wunsch auf Elternschaft erfüllt sondern als die gesollte Erfüllung eines normativen Bedürfnissanspruchs begriffen werden muss. Der Beitrag beschäftigt sich mit der Frage, ob eine Behinderung immer als Form von Leid betrachtet werden muss. Mit Hilfe einer Unterscheidung zwischen absoluten und komparativen Einschränkungen des Wohls wird aufgezeigt, dass die bloße Tatsache einer vorliegenden medizinischen Schädigung nicht hinreicht, ein Urteil über das absolute Wohl einer Person zu treffen. Es werden verschiedene Argumente geprüft, warum Behinderung dennoch generell negativ bewertet werden sollte. Diese werden zurückgewiesen. Abschließend wird eine Überlegung eingeführt, wonach gleichwohl bestimmte Formen der Behinderung als objektive Beeinträchtigungen (...) des Wohls zu gelten haben, nämlich dann, wenn basale Fähigkeiten betroffen sind. The placebo drug reactions from controlled trials were studied for the first time systematically for efficacy and the safety in drug data pooled from randomized, placebo-controlled, multicentre studies. Results: The efficacy of placebo on clinical symptoms and outcome varied between the therapeutic indications. However, no placebo effects on laboratory values, as e.g. blood glucose or Hb1c in diabetics, were noted. The frequency and type of placebo-induced adverse reactions also varied between indication groups. The placebo side effect profile was largely similar (...) to the side effect profile of the active treatment. The mechanisms of placebo effects are manyfold and varied (e.g. endorphin release, conditioning), much lacks explanation. Conclusion: Since the prescription of non-evidence based medicines (= pseudoplacebos) may clearly also result in serious adverse effects, such practice may not only be non-beneficial but may even be harmful. In clinical research, the judicious use of placebo remains essential to establish the efficacy and safety, safeguarding that patients receiving placebo will not be subject to harm and are fully informed. The Sneetches by Theodor Geisel (otherwise known as Dr Seuss) is a satirical story that targets illicit discrimination. The book presents its parable about discrimination by depicting a society in which one group discriminates against another group because of an easily perceptible difference between them. The real irrationality of discrimination in both The Sneetches and real life is that it is based on the false claim that members of the discriminated‐against group are inferior to members of the discriminating group. The (...) book ends with all of the Sneetches playing with one another, paying no attention to the presence or absence of stars on their bellies. The lesson that children draw from The Sneetches is clear: Discriminating against people in a different social group is wrong. (shrink)

Paul Johnston demonstrates that much recent moral philosophy is confused about the fundamental issue of whether there are correct moral judgements. He shows that the standard modern approaches to ethics cannot justify - or even make much sense of - traditional moral beliefs. Applied rigorously, these approaches suggest that we should reject ethics as a set of outdated and misguided claims. Rather than facing (...) up to this conclusion, most recent moral philosophy consists of attempts to find some ways of preserving moral beliefs. This places a contradiction at the heart of moral philosophy. As a resilt it is often impossible to tell whether a contemporary philosopher ultimately rejects or endorses the idea of objective right and wrong. On the basis of a Wittgenstein approach Paul Johnston puts forward an alternative account of ethics that avoids this contradiction and recognises that the central issues of ethics cannot be resolved by conceptual analysis. The primary aspects of Clifford Christians's ethical theory may be identified or contextualized in several ways, three of which are employed in this article: 1) a content analysis of his self-reported book, article, and chapter titles; 2) a narrative summary of the themes of his self-selected representative ethical theory essays; and 3) the author's contextualization of Christians' ideas within both intellectual history and communication studies. Although Christians and his work are valued as apex contributions to and leadership within the field (...) of media ethics, three questions are posed to Dr. Christians to which he responded both at the August 2009 AEJMC conference panel in Boston about his intellectual legacy and again in this journal. This book builds on the vast clinical experience of Joseph L. Henderson, who became interested in initiatory symbolism when he began his analysis with Jung in 1929. Henderson studied this symbolism in patients' dreams, fantasies, and active imagination, and demonstrated the archetype of initiation in both men and women's psychology. After Henderson’s book was republished in 2005 Kirsch, Beane Rutter and Singer brought together this collection of essays to allow a new generation to explore the archetype of initiation. _Initiation: The (...) Living Reality of an Archetype_ demonstrates how the archetype of initiation is seen clinically today. Divided into distinct parts, the book explores the archetype of initiation in Dr Henderson’s own life, as well as suggesting its importance in: clinical practice culture aging and death. The chapters in this book amplify and extend the archetype of initiation from the earliest historical periods up to the present day. The editors argue that initiation symbolism often underlies contemporary phenomena, but is rarely recognized; _Initiation _helps to bring a new understanding to these experiences. For a long time Hume's philosophical achievement was judged almost entirely by Book I of the Treatise. A major contribution of Kemp Smith's work on Hume was the insistence that the epistemological doctrines of Book I were essentially related to the ethical theory of Book III. Recent moral philosophy has found Book III to be of considerable intrinsic interest and relevance to current problems. It is now becoming apparent, however, that Hume's ethical theory is intimately bound up with the philosophy (...) of mind in Book II. This new emphasis upon the significance of Book II for Hume's ethics derives from Dr Pall S. Árdal's book Passion and Value in Hume's Treatise. In view of Árdal's influence on continuing study in this field—witness the recent book by Philip Mercer—it becomes important to point out what I take to be a major error in one part of Árdal's analysis of the relationship between Hume's philosophy of mind and his moral theory. It shows much acuteness, great precision and elegance of expression, bat considerable repetition, and often appears as one turning in a circle.
